Apartments, cottages, homes or rooms that can include one or more of the following supportive services: meals, housekeeping, some assistance with personal care, transportation, socialization activities, laundry, resident/service coordinator who helps residents access other services and programs from community-based agencies. Personal/home care services through a licensed home care agency are usually available at an extra cost.

Payment Options: Private pay –typically $2000 - $5000/mo.  Residents may also purchase personal/home care services through licensed agencies, which can be paid for privately, or may be covered by long term care insurance, health insurance, or Medicare/Medicaid. Check with each individual facility for payment options.

A long-term care option that combines housing, support services and health care, as needed. Assisted living is designed for individuals who may require assistance with everyday activities such as meals, medication management, bathing, dressing and transportation. Some residents may have memory disorders including Alzheimer's, or they may need help with mobility, incontinence or other challenges.

Payment Options: Private pay for monthly rents that range from $1000 to $8000 a month. Some of the newer long term care insurance policies include an assisted living benefit or will cover home or personal care services provided by a home health care agency. Check with each individual facility to determine if Medicare/Medicaid payments are accepted.

A skilled nursing home provides 24 hour a day skilled nursing care and related services, or rehabilitative services for the injured or disabled.

New York State's nursing homes are regulated by the State Health Department.
